Responsibilities: Provide skilled nursing care, including G-tube feedings and tracheostomy care Monitor and document client health status and report changes as needed Administer medications and treatments per physician orders Collaborate with the healthcare and direct support teams to ensure safe, person-centered care Ensure compliance with DDI and state regulatory requirements Knowledge & Abilities Strong clinical assessment skills and the ability to develop and implement care plans. Excellent communication and interpersonal skills. Proficiency in managing and administering medications. Ability to work independently and as part of an interdisciplinary team. Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite. Requirements: Active LPN license in MD Proven experience with G-tube and trach care Previous experience working with individuals with developmental disabilities (DDI) Strong documentation and communication skills Reliable, professional, and compassionate Shift
• Monday – Friday 9am – 5:00pm and flexibility to accommodate individual needs and schedule variations. We are seeking a compassionate and experienced Level 3 Licensed Practical Nurse (LPN) with DDI experience to provide high-quality care in a residential or community-based setting. The ideal candidate will have hands-on experience with G-tube feedings and tracheostomy care and a strong commitment to supporting individuals with developmental disabilities.
