Looking for an enthusiastic couselor who is eager to work alongside a great team of providers in a Family Practice setting! Responsibilities: Conduct thorough patient assessments and work collaboratively to establish personalized therapy goals that spark meaningful progress and sustainable recovery. Deliver impactful individual and group therapy sessions while maintaining meticulous, confidential records that uphold clinical excellence and quality care standards. Track and celebrate meaningful therapy progress by evaluating intervention effectiveness and strategically adjusting your approach to drive optimal outcomes. Empower patients by connecting them with community resources and therapeutic activities that extend healing beyond the counseling room and strengthen their daily lives. Collaborate seamlessly with patients, families, physicians, and community partners to deliver integrated, whole-person care—from initial assessment through successful discharge and ongoing support.
