Case Manager

This position works to ensure the timely and seamless clinical transition of patients through medically appropriate levels of care. Uses skilled interventions and evidence-based practice to collaborate (patient, physician, and multidisciplinary treatment team) and achieve quality outcomes, paces care and negotiates appropriate reimbursement. Iowa RN licenseBachelor’s Degree in Nursing or related field preferred, as is Case Management Certification.
